Physical therapy is a type of treatment you may need when health problems make it hard to move around and do everyday tasks. It helps you move better and relieves pain. It also helps improve or restore your physical function and your fitness level. 

The goal of physical therapy is to make daily tasks and activities easier. For example, it may help with walking, going up stairs, or getting in and out of bed.

More than half of all Americans are suffering from pain. Whether it is a recent episode or chronic, an ABC News/Stanford study revealed that pain in America is a serious problem. However, many do not even know that physical therapists are well equipped to not only treat pain but also its source.

Physical therapists are experts at treating movement and neuro-musculoskeletal disorders. Pain often accompanies a movement disorder, and physical therapists can help correct the disorder and relieve the pain.

Massachusetts is an "open access" state. This means that if you would like to have physical therapy, you are welcome to make an appointment. However, Medicare does require a primary care physician or specialist to write a orders for treatment. If you are a Medicare patient and do not have a prescription from a doctor, please call your doctor and tell them you'd like to see us for physical therapy!

Your insurance plan may also require a referral from your doctor. This is different than doctor's orders/prescription, which, just like a prescription for medication, is written by the doctor and given to our office. A referral is a notice from your primary care doctor to the insurance company that you will be attending physical therapy. If you are unsure if you need a referral, contact your insurance company to discuss your benefits.

Your first visit will last approximately one hour and will include a comprehensive evaluation and applicable treatment administered by a licensed physical therapist. Your physical therapist will review your medical history, diagnostic tests and any recent events that may have contributed to your current condition. Your evaluation will include assessment of your current functional deficits, pain level and posture, as well as a thorough evaluation of your flexibility, strength, balance and endurance.

Following your evaluation, your physical therapist will thoroughly explain your personalized treatment plan and expectations for recovery. Communication between the patient and physical therapist is of utmost importance at Evolution Physical Therapy and you are an active participant in your recovery.

If you are covered by Workers' Compensation, bring your claim number and your case manager's contact information. If you are covered by auto insurance or an attorney lien, make sure you bring this information.


Comfortable clothing, athletic wear if possible, preferably no jewelry.  Area of concern should be readily accessible.  We gladly provide treatment gowns for ease of access where necessary.  

Please arrive 15 minutes before your first appointment in order to complete all necessary paperwork prior to your scheduled appointment.

Duration and frequency of treatment are unique to each individual’s injury or condition. Evolution Physical Therapy professionals are dedicated to returning patients to optimal health. The cost of your customized treatment plan depends on your diagnosis, individual needs and presence of and type of insurance plan. If you have insurance, we will help verify your insurance benefits before beginning treatment. Each insurance plan is different. Your plan may have a deductible, a coinsurance, or a copayment. We ask that you please call your insurance company to understand what they will and will not cover. To request patient records, please submit to our office manager a completed Authorization for Access, Use and/or Disclosure of Protected Health Information form ( attach ) and the specific information needed for the request.

A $15 fee may be charged, at the discretion of the office manager, pending time requirements to retrieve and volume of requested record.
